Verona - Hospital Discharges


From UW:

When a patient on your panel is discharged from UW, you will get the Discharge Summary (you may actually get several copies in your InBasket, depending on who forwards it to you).

It is your responsibility to review “Follow up issues” in this summary and arrange them.

You may also get a separate InBasket message with recommended discharge follow up orders, which have been specified by the inpatient team and triaged by a UW nursing pool.

Lastly, it is a shared responsibility of the discharging team and you to update Problem Lists and medical history.

From outside hospitals:

When a patient on your panel is discharged from another hospital, you will get a discharge summary. Again, it is your responsibility to triage follow up issues, reconcile medications, update problem lists, and order follow up labs.
