DISABILITY BENEFITS
Licensed Employees are automatically provided Long Term Disability (LTD) insurance at employee expense with Standard Insurance Company through OEBB. Benefits begin after a 90 day waiting period, the policy pays 60% gross wages. Employees may purchase Short Term Disability (STD) coverage through two insurance companies: Standard Insurance Company with OEBB enrollment or AFLAC. Employees should carefully consider whether they need short term disability coverage if they have more than a 60 day sick leave balance remaining with the district.

LIFE AND ACCIDENTAL DEATH AND DISMEMBERMENT
Employees may purchase optional life insurance when first eligible for coverage through the district. Optional accidental death coverage is available at every Open Enrollment.
Standard Insurance Company - Enroll during OEBB open enrollment
Medical History Statement -required if requesting coverage over the guaranteed coverage amount below.
Employees may purchase up to a $200,000 Optional Life Insurance without providing proof of medical insurability when first eligible for coverage (newly hired.) During subsequent annual enrollment periods, OEBB members may request an increase in smaller increments and are required to provide medical proof of insurability. Up to $500,000 optional accidental death and dismemberment coverage may be purchased during any open enrollment period without medical documentation.
Employees may purchase up to $30,000 Optional Life and AD&D limit for spouse/partner without providing proof of medical insurability when first eligible for coverage (newly hired.) Coverage may purchased in an amount over $30,000 to maximum $200,000 after medical documentation is provided.
Employees may purchase up to $10,000 Optional Life and AD&D for children when newly eligible for coverage.

LONG TERM CARE INSURANCE
Unum Insurance Company - Enroll during OEBB open enrollment
The Oregon Educator's Benefit Board (OEBB) contracts with UNUM Insurance Company to provide Long Term Care Insurance to member districts Long Term Care Coverage is available to all benefit eligible employees, participation is voluntary and at employee expense. Family members of employees are also eligible for long term care coverage. Family coverage is medically underwritten.
