HOW MUCH DOES THE DISTRICT CONTRIBUTE?

TWO MARRIED EMPLOYEES MAY COMBINE CAP

If either licensed or administrative employees are both employed by the district and are either married or qualify for insurance based on their status as "domestic partners" they may participate in the following arrangement with the district for the 2024-25 plan year: 

Complete and sign the 2024-25 employees agreement to combine insurance CAP  form and return to the Payroll Department.

OPT OUT

CCSD employees are eligible to "opt out" if they maintain coverage under another employer-sponsored group medical benefit plan. (this does NOT include plans such as OHP).  Employees who have chosen to "opt out" will receive a stipend of 30% of their eligible cap in their regular paycheck per month.  During open enrollment or when new employees are first eligible for coverage, employees may complete the Opt Out Form 2024-25 and return to the payroll department or the Benefits Department by e-mail or fax 541-447-3645.
