What do I need to do when I need to transition a client to another therapist?

After you have approval from your manager to transition a client, please submit the Single Transitions Form

Also, prior to submitting the form, not only do you need your manager's approval, you need to notify the family regarding the planned transition. Please leave a note in child's chart indicating you have requested a transition and have notified the family of request.

You are responsible for continuing to provide services for this child until we are able to find another great match for the child.

Requests will be considered for in-person services unless telemedicine or hybrid is specified.

What do I need to do when I need to transition a caseload, i.e. heading out on Maternity Leave?

First, please speak with your Manager and they will assist you to get this process started. After speaking with your manager, you may be asked to fill out the Caseload Transition Form.
