How do I complete a discharge?
Discharge Process
CLINICAL:
Clinician or family identifies D/C
Clinician completes the DISCHARGE FORM (open from link or in section below)
Please note whether or not a final DC visit is taking place.
Add communication notes to the chart, Client Profile > Notes, to keep all team members informed.
Clinical Documentation:
For discharges with a visit, you will complete a regular routine visit note AND the "COFK SLP/OT/PT DC" form/note that office team adds to your schedule as non-billable. Times should not overlap, and the non-billable note could be for 1 minute, since not billing.
For DCs, without a visit, ONLY the non-billable DC Form is needed.
OPs:
Ops staff receives notification, schedules unbillable DC Summary Form on D/C date
Remove visits, if applicable, after D/C date
Complete 5 day notice, send by mail, to family
Ops staff creates D/C order upon completion/closed visit & sends to MD (no signature required)
Ops staff changes discipline admission status to “Discharged”
