Incident and Infection Reports
When should Incident Reports be submitted?
Any type of patient injury occurring during a visit, even minor falls, scratches, etc.
Any type of therapist injury occurring during a visit.
Reports made to CPS website or patient’s assigned CPS case worker.
Animal bites to therapist or patient during visit or immediately before or after the end of a visit. (Even if the skin is not broken)
Suspected Neglect or Injury to patient from family members or daycare staff.
Behavioral issues with patient during visit (biting, kicking, hitting, spitting, pinching, cursing, throwing objects, hair pulling)
Inappropriate behavior from family/guests in patient home
Unsafe activities occurring in home (Drug usage or suspected drug sales, family member/guest under the influence of drugs or alcohol)
Unsafe environment outside the home/apartment
Damage to therapy materials/electronics
Other
Patient Incident Note Types are available in Cubhub and should be used for incident reporting.
When should Infection Reports be submitted?
You should complete an Infection Report note as soon as possible and no later than 24-hours of being notified of the illness.
For example:
Your last visit was on 1/18 and your next scheduled visit is on 1/23.
On 1/23, you are notified by the parent that the child developed symptoms on 1/21.
You would submit your Infection Report note on 1/23 and note 1/21 as the date of the infection.
Submitting your Infection Note no later than 24-hours of learning of the infection is critical. Many illnesses, such as COVID-19, have board of health reporting requirements in place.
Infection Note Types should be used for reporting infections.
