Hospitalization - Holds & Resumption of Care 

How do I put a client on hold when hospitalized? 

To start a Hospitalization Hold

a. Clinicians will receive a form receipt via email.

b. Clinicians can expect a CH inbox message within 24-48 hours confirming hold processing from the Ops/HIM team.

How do I restart care when the client is discharged from the hospital? 

To resume care when child is discharged

a. Ops Team will confirm receipt of resumption orders.

b. Ops Team provides green light notification to clinician to resume care.

c. Ops Team will update client visits to have billable service codes.

a. Use a regular routine visit form.

b. Include parent report of hospitalization and MD updates.

c. Address all goals.

d. Add communication note to chart for Resumption of Care (ROC) visit.

e. If goal updates are made via LPR (in the Plan section) it requires a POC Update Change Order

f. NOTE: If authorization expired during the hold, complete the full Re-Eval Form/Note. Re-eval covers all POC updates, new orders, and signals a new POC.
