EVV Compliant Daily Visit Notes & Daily Routines
(Missed Visits, Make-Ups, Calendar Reconciliation)
Electronic Visit Verification (EVV) has started in Texas as of 01/01/2024 per Federal mandate.
When there is an EVV error, your session note is not billable until corrected. Please see the resources above for additional details/definition of EVV.
In effort to minimize EVV errors and ensure your session notes are billable, we ask for all therapists to ensure the following are always completed accurately.
Actions Required by All Therapists:
Clock In: Completed at the start of the session with the patient present. Ensure the date, time, AND service location are accurate.
This is important as your geo-location is tagged at time of clock-in.
Clock Out & Obtain Parent/Guardian Signature: Completed at the end of the session with the patient present. Ensure the date and times are accurate. Remember it is a 24-hour clock.
This is important as your geo-location is tagged at time of clock-out.
Confirm Service Location: In/Out Service Location must be the same at clock-in & clock out. Ensure you select the accurate service location category at every session. The location chose at the start is particularly important, as it pulls through to the end. The service location is one of 5 categories - Member Home, Community (daycares), Family Home, Neighbor, or Telehealth.
When asked: “Is this visit being performed via Telehealth?” - answer accurately.
Service Code Accuracy: Check and confirm at Therapist sign-off. This is important as it must match the Service Location you selected at clock-in/out.
If completing a Telehealth session, use a Telehealth (TH) Service Code.
If completing an In-Person session, use an In-Person Service Code.
Completing these steps during the patient encounter will greatly minimize errors.
