Goal Progress Summaries & 60-Days

Currently (Jan 2024), the process is as follows:

• Complete a full visit note/form (billable IF a visit occurred, or as an Unbilled Segment/Non-Billable if NO visit occurred)

• Comment on the progress of ALL goals comparing current levels with baseline levels

• Include attendance rate, completed visits, missed visits w/reason why, and overall compliance with POC

• Compliance with HEP strategies

• Any updated restrictions or that there are none

• Recommendation for ongoing services (freq & duration) & objective statement about why continued therapy is recommended/justified

• IF you need to change or update goals, please do so via the Live Patient Record (LPR), Plan Section > Goals, for your discipline

  • Include baseline data with new goals

IMPORTANT: After completing Goal Progress Summaries/Goal Updates and 60-Days that have changed/updated goals (require pediatrician signature), please email pf.qa@careoptionsforkids.com (COFK-S) or processing@cofk.com (COFK-N) letting them know the name of the patient and if it’s a goal update or 60 day needing pediatrician signature. Please cc: your Area Manager or CSM.
