Signing Authorization (CCP) Forms
In order to get recent evals & re-evals sent out for signature & auth, you might be asked to sign an authorization (CCP) form in Cubhub.
These forms can be found in the Documents section of the chart. Click the document on the left or the link HERE for a step-by-step guide.
Who do I contact if I have questions about the status of an authorization in CH? For example, visits that are scheduled as "UNBILLABLE", do I have authorization to complete these visits?
Please reach out to CHauths@pathfinderpediatrics.com with any questions about your client's authorization status or whether or not it is ok to to complete visits that are scheduled as "unbillbale"?
Unbillable does typically indacate that we do not have authorization for that visit, so it should not be completed.
