I formally started the structured ROJoson Telemedical Consultation on March 17, 2020.
I do telemedical consultation with me and my patients staying at home using virtual technology.
For the patients whom I have seen before in my clinic in Manila Doctors Hospital and who did telemedical consultation with me for follow-ups and check-ups, I have found it extremely helpful in making an assessment when they could locate the medical records or my medical notes that I issued to them before in my clinic. I asked them to take a picture of the medical records and show them to me online.
Their previous medical records or medical notes are in my clinic in Manila Doctors Hospital. I cannot access them from my clinic in this time of enhanced community quarantine against COVID19. The patients are supposed to keep a duplicate that I have always issued to all patients who consulted in my clinic. (Note: I am probably the only physician in the Philippines who gives a duplicate copy of my clinic notes or records to patients - ready to be refuted though). I have been doing this as early as 2005 or even earlier.)

In my clinic (ROJoson Medical Clinic), I have the practice and habit to give all my patients a duplicate or carbon copy of my notes and explanations.
I am still trying to trace when I started doing this, probably circa 2000. Below is a copy of a 2005 record that I was able to retrieve. I will keep on tracing the time I started this practice.
I usually ask my patients to sign the medical records that I made for them, either as part of informed consent or refusal for my recommendations on management or simply, for acknowledgement purposes.
I also usually ask my patients to always bring their past medical records whenever they come for check-up or follow-up with me.

Up to now, continuing my habit in face-to-face medical consultation in my ROJoson Medical Clinic, after each telemedical consultation, I still give a copy of medical record to all my patients. After the video consultation, I will make a record on my pad first (handwritten), then take a picture and then send it to my patients online, either through email or through FB Messenger.
