I, Dr. Reynaldo O. Joson, have been doing telemedical consultation when I started private practice in 1981 through telephones, pagers (Pocketbell, Easy Call), cellphones, emails, and through facebook (starting June 16, 2011) and Viber.
I decided to formalize it into ROJoson Telemedical Consultation starting March 17, 2020 when enhanced community quarantine in Luzon was imposed because of the COVID19 crisis.
I have to stay home and patients have to stay home. I have to work from home to treat my patients and the only way is through a TELEMEDICAL CONSULTATION.
ROJoson's Way (objectives / intentions)
To preserve continuity of ROJoson Medical Clinic services to the Filipino people.
To protect the Filipino patients from being put at risk of COVID-19 infection when they go to ROJoson Medical Clinic for consultation because of the COVID-19 epidemic.
To offer online medical consultation services to the Filipino people who cannot or have difficulty going to clinics and hospitals because of the community quarantine and who have no physicians to consult because of closure of clinics during the COVID-19 epidemic.
Telemedical Consultation is not a face-to-face medical consultation where the physician directly sees and touches a patient to evaluate his / her medical concern, then to make a clinical diagnosis and then recommendations for treatment.
In Telemedical Consultation, interactions between the physician and patient for purposes of medical evaluation and medical recommendations are made through information technology or gadgets such as telephone, cellphone, email, Facebook, Viber, Hang-outs, Zoom, and other online apps with no face-to-face interaction.

What is a Medical Consultation?
Medical consultation, whether tele (virtual) or face-to-face, is defined as a patient or a relative of the patient voluntarily seeking out a physician to get professional ADVICES on certain medical concerns.

ROJoson Telemedical Consultation
The physician, as a matter of basic routine, interviews and does physical examination on the patient to get to a clinical diagnosis. If the patient has previous medical records, these are reviewed with the interview and physical examination data to get to a clinical diagnosis.
After the clinical diagnosis, the physician explains his clinical diagnosis to the patient or relative and then advises on the subsequent steps in the management. The advice on subsequent steps of management may include medications, diagnostic and treatment procedures, or just a watch and wait or monitoring approach.
A medical consultation strictly speaking ends on the physician giving advice. It does not include prescription of medications and performance of diagnostic and treatment procedures on the patient. However, prescription of medications and request for diagnostic procedures can be included in a medical consultation usually with no additional or extra service charges to the patients.
Performance of diagnostic and treatment procedures is entirely separate from medical consultation and carries additional service charges to the patients.
There are a lot of telemedical consultation services being offered by various physicians, medical clinics and hospitals.
How ROJoson Telemedical Consultation differs from other telemedical consultation services
ROJoson's Telemedical Consultation approximates ROJoson's conventional Face-to-Face Medical Consultation!
ROJoson Telemedical Consultation approximates the standard ROJoson's face-to-face medical consultation with patients in his ROJoson Medical Clinic.

- Reviewing patient's previous and past medical records
- Doing medical recording in a stationary or digital platform (digital document, white board, paint board, etc.) which contains primary clinical diagnosis; explanations; recommendations
- Discussing with patient to come out with shared decision-making and informed consent on treatment plan
- Educating patients
A scanned or digital medical record will be given to the patient. Patient shall acknowledge understanding and receipt of the medical record.
ROJoson takes time to interview, ask and gather data meticulously.
He takes time to examine patients meticulously using physician-guided with patient-relative-assisting physical examination aided by still pictures and videos to promote accurate findings.
He patiently takes time to teach patients to empower them on how to diagnose medical conditions and diseases and how to do physical examination on themselves which is needed for self-monitoring purposes after the consultation.
He always gives his primary clinical diagnosis followed by clear and extensive explanation, recommendations and discussions on subsequent steps to take and what to expect after the consultation.
He takes time to discuss with patients and relatives to come out with shared decision-making and informed consent on treatment plan.
He provides patients with a digital medical recording of what transpired during the telemedical consultation for them to review and to keep for future reference.

Policies:
ROJoson Telemedical Consultation services include initial screening of concerns even if at first glance, they are not within the specialty of Dr. Joson.
After evaluation, proper referral to competent and compassionate specialists is facilitated by Dr. Joson, either for another telemedical consultation with the referred specialist or face-to-face medical consultation.
Looking for the ROJoson's preferred specialists is part of ROJoson Telemedical Consultation.
Thus, there is a consultation fee involved. Fee will depend on complexity of case; length of telemedical consultation; degree of difficulty of referring to preferred specialists, etc.

Some friends on Facebook assume they can consult me anytime any case on Facebook for free.
