Evaluation Of case management Model Hennepin County Developmental Disabilities Program (2005) — The Institute on Community Integration, University of Minnesota was asked to review case management literature and consult national experts about case management and support coordination, and provide an outside perspective concerning the design of the new case management model: whether the design was workable and effective to serve growing numbers of clients with fixed case management resources.

Evidence-Based Practice for Social Work: Friend or Foe? (2013) — The phrase ‘evidence based practice’ has also become corrupted over time. In 1996 Sackett et al. defined evidence-based medicine as “the conscientious, explicit, and judicious use of current best evidence in making decisions about the care of individual patients. The practice of evidence-based medicine means integrating individual clinical expertise with the best available clinical evidence from systematic research”. In 2000 this definition was revised to “the integration of the best research evidence with clinical expertise and patient values” (Sackett et al. 2000)

Integrated Services for People with Long-term Neurological Conditions: Evaluation of the Impact of the National Service Framework (2010) — The successful management of LTNCs, in order to enhance health and well-being, needs both sophisticated management across a number of health, social care and other service boundaries, and involvement of people with LTNCs and members of their support networks. Intensive Case Management Service Standards for Mental Health Services and Supports (2005) — Intensive case management promotes independence and quality of life through the coordination of appropriate services and the provision of constant and on-going support as needed by the consumer.

Multidisciplinary vs. Interdisciplinary Teamwork: Becoming a More Effective Practitioner 2014 — Both multidisciplinary and interdisciplinary teamwork have been used in healthcare in the past. However, although there is a clear difference between the two approaches to care, many educational researchers and practitioners use these terms interchangeably. The fundamental difference lies in the collaborative care plan that is only developed in interdisciplinary patient interventions, as multidisciplinary care does not emphasize an integrated approach to care.

The review was part of a larger project to support the development of a benchmarking tool that could be used to evaluate progress in local health and social care communities in developing integrated care for LTNCs. Redesigning case management Services for People With Disabilities in Minnesota (2007) — The Institute on Community Integration at the University of Minnesota prepared this report on proposed models for reforming case management.
