The Clinic uses an electronic medical record system called Titanium Schedule. Each counselor is responsible for ensuring that the following has been completed in their client's digital chart.
Practicum Counselors:
Completed Clinical Case notes for each attended session written in clinical language with no spelling or grammatical errors.
Ancillary Notes for unattended sessions or any outside-of-session client contact, written in clinical language with no spelling or grammatical errors.
Any relevant "Data Forms," which include the Informed Consent (first session), Service Plan (third session), Service Plan Review (10th session or halfway through service), Final Service Plan Review (fourth-to-last session, or at the client's notice of intent to terminate), and the Termination Summary.
Forwarding notes to Practicum Instructor for feedback & signatures
Intern Counselors (the above, and also):
Rescheduling client sessions - If a client would like to shift appointment times, consult with the Clinic Coordinator first
Billing & invoices for each client including cancellations/no-shows
Completing Intake Interviews for incoming clients
Creating new client charts in Titanium Schedule
Practicum Instructors:
Reviewing all notes regarding your supervisee's clients and signing (On Spot 3) once they have been completed to your satisfaction
Marking attendance of sessions, if counselor has not done so
Billing & Invoices for each client
Submitting end-of-day Revenue Summaries for Practicum section to Clinic Coordinator
