The Community Counseling Clinic (CCC) recommends counselors inform clients of pending terminations 2 - 4 weeks before the termination date. When terminating services is anticipated and consequent to a successful counseling plan, all CCC counselors are responsible for completing the Termination Summary along with a Clinical Case Note. Termination summaries must be completed at the time of, or promptly after termination, and should not be postponed until a later date. Termination summaries must be fully completed before the CCC counselor will receive a new client.
All CCC counselors in practicum are required to complete a termination summary at the end of the first section of practicum for all their clients. All CCC counselors in internship are required to complete termination summaries at the conclusion of their internship.
Termination Procedures:
2-4 weeks prior to termination, complete the Final Service Plan Review.
Complete a Termination Summary Data Form attached to the final Clinical Case Note or Ancillary note.
Review the client's "Client Chart" in Titanium to ensure documentation is corresponding with all appointments.
Write a communication slip (or email) to the clinic coordinator to make sure that the termination date is clear. Indicate that you are available to see a new client. If your Termination Summary is not found fully complete in the client's chart, you will not be assigned another client until it is complete.
