Community Counseling Clinic (CCC) counselors must maintain a client Service Plan. Service Plans must be completed on or immediately following the third session with your client(s). The counselor is responsible for regularly engaging and updating the Service Plan, as needed. Formal service plan revisions will be expected during the course of counseling services.
CCC counselors will be expected to complete the 1st Review of the Service Plan after 10 sessions with clients or midway through counseling.
CCC counselors will be expected to complete the Final Review of the Service Plan 4 weeks prior to the end of counseling or immediately following a client's notification of intent to terminate.
Crafting the Service Plan:
The CCC counselor(s) must indicate the presenting concern as understood by the counselor after three sessions. "Presenting Concern" options are in a drop-down menu, and the counselor ought to check in with the client regarding their selection.
The CCC counselor(s) must provide a Clinical Impression at the creation of the Service Plan, using information gathered throughout early sessions. Clinical Impressions can be crafted as a clinical hypothesis or the expected etiology of client presenting concerns.
Clinic Impressions ARE NOT A RESTATEMENT OF THE PRESENTING CONCERN. Counselors should consult with their Practicum Instructors for more information.
The CCC counselor(s) must review and sign the Informed Consent at the start of services. The Service Plan should indicate that this was successfully completed.
The CCC counselor(s) must review the Intake Interview and ensure that the document has been fully completed. Missing information must be obtained from the client.
(For Telehealth sessions, counselors will review Service Plans via screen share, then sign the SPs themselves.) Client signature of the Service Plan is mandatory and must be obtained after the completed Service Plan is reviewed with the client. CCC counselors cannot obtain client signature and complete the Service Plan post-session.
1st Review of the Service Plan:
The CCC counselor(s) can indicate whether client goals have been 'completed,' 'continued,' or 'revised' with notes in the "Notes at 1st Review" section OR describe progress in the Notes at 1st Review".
Counselors should consult with the Practicum Instructors regarding their preferences for Service Plans.
The CCC counselor(s) must present a revised clinical impression at the 1st Review on the "Impression at 1st Review" line, or indicate "same as above" if clinical impression has not changed.
Clinic Impressions ARE NOT A RESTATEMENT OF THE PRESENTING CONCERN
The CCC counselor(s) are expected to initial the Service Plan at 1st Review (with client initialing "Client Initials at 1st Review" - once we return to in-person care).
Final Review of the Service Plan:
The CCC counselor(s) must indicate whether client goals have been 'completed,' 'continued with continuing services,' or 'continued with outside services,' or 'incomplete with client consent for termination' OR describe progress in the "Notes at Final Review."
Client plan for 'incomplete' goals should be indicated in additional comments at Final Review and in the Termination Summary.
Counselors should consult with their Practicum Instructors regarding preferences for final review.
The CCC counselor(s) must initial Service Plan (with client prior to termination - once we return to in-person care).
