Community Counseling Clinic (CCC) counselors are responsible for completing a Termination Summary at the end of their work with all clients, without exceptions. Termination summaries permit the coordination of continued care or document that services are no longer needed. See Clinical Procedures for the procedural requirements in closing a client chart. While they will assist you by sending the Letter of Termination, the Clinic Director will not be responsible for completing the Termination Summary. Then can provide counselors with referral information for outside services as needed.
Termination Policy:
The Termination Summary must be completed immediately following the final decision to terminate or on the date of termination. They should not be done at a later date.
Termination Summary forms must be fully completed before counselors will be scheduled new clients.
CCC counselors must inform the Clinic Director of impending or completed terminations.
CCC terminations are conducted with consent from clients, unless consequent to violation of cancellation and no-show policies (see below).
Terminations with only one or two attended sessions do not require the completion of the Service Plan. However, counselors ARE expected to complete the Termination Summary.
Terminations consequent to recurring cancellations or no-shows proceed in accordance to the following policy:
Clients with recurring cancellations with notification may be terminated at the approval of the Clinic Director, and only following consultation with the Practicum Instructor, Clinic Coordinator, and the CCC counselor.
Terminations resulting from recurring cancellations must follow contact between counselor and client informing them of termination, and include a Letter of Termination sent by the Coordinator. CCC counselors are responsible for the completion of a Termination Summary indicating the date of last contact.
Clients with three or more consecutive' no-shows' must be informed of the intent to terminate and be provided the opportunity to reengage services. For "No-Show" termination processes go to Cancellation Policy.
Clients who contact the clinic prior to termination, but do not keep sessions immediately following contact, may have their charts closed and services terminated. CCC counselors and Clinic Coordinator must inform the client of this policy at point of contact.
