What time will a closure be called?
NKY Health will make every effort to call closures for inclement weather by 6am. However, some closures may be called outside of that time due to unforeseen conditions that are not reasonably predictable, such as a water main break or weather turning intense later in the day.
How will I know if my site is closed?
A message will be sent to your fname.lname@nkyhealth.org email account
If you have opted in to text messaging, you will also receive a text to the phone number you provided.
If you haven't opted in to text messaging yet, you can do so by texting the word CLOSURE (all caps) to 833.693.3992 (the way you normally send a text message.)
All messages will be sent as soon as possible after a closure is called.
Who is paid during a closure for unforeseen circumstances or inclement weather?
Only employees who are “scheduled to work” or "required to work" (Refer to section 3.7.1 of of NKY Health's Personnel Policies) will be paid during an unforeseen closure period. Employees scheduled to be off will code their time as if the closure had not occurred.
How do I know if I am required to work during a closure?
Each division director will determine who is "required to work" (Refer to section 3.7.1 of of NKY Health's Personnel Policies) during a closure and will explicitly notify you if you are required to work.
Division directors will also email a list of those employees who are "required to work” to payroll@nkyhealth.org.
What if I am required to work?
If you are exempt (salaried), then the number of hours equal to those you worked will be added to your Annual Leave bank in ADP the next time payroll is processed.
if you are non-exempt (hourly) then you will be paid double time for the hours you were required to work during the closure period.
What if my normal work day starts before a closure is called?
If you have an alternate schedule, please confer with your supervisors when inclement weather is forecasted. When a closure is reasonably predictable, make arrangements to alter your work schedule or to work from home if your supervisor tells you you are "required to work."
if you work before a a closure is called, you will be paid as normal for the hours you worked outside the closure. The balance of their scheduled work day will be paid as closure time so the timecard reflects your normal number of scheduled hours.
How should timecards be coded for closures?
If you were “scheduled to work,” (Refer to section 3.7.1 of of NKY Health's Personnel Policies) you should code the number of hours you were scheduled to work that cut across the closure period, up to 8 hours, to the Closure pay code in ADP and add a note to your timecard stating the reason for the closure.
If you are “required to work” (Refer to section 3.7.1 of of NKY Health's Personnel Policies) you should also code the hours you worked as you would normally code them in ADP and add a note to your timecard stating that you were required to work.
NOTE: Coding hours to both “closure” and a cost center will trigger double-time pay for non-exempt employees and additional annual leave for exempt employees.
If you are “authorized to work” and “scheduled to work” but not “required to work” (Refer to section 3.7.1 of of NKY Health's Personnel Policies), you should code time the way you would normally code it. You will not be paid double time or have time added to your Annual Leave bank.
How do I add a child or spouse to my insurance?
You can add a dependent to your insurance during open enrollment without needing any documentation.
Outside of open enrollment, you must experience a qualifying event in order to add them. Please refer to the "How to Make Changes Outside of Open Enrollment" section of the Group Health, Dental, and Vision Insurance Benefits page of this site for details.

How do I drop my OWN insurance?
You can drop your health insurance during open enrollment.
Outside of open enrollment, you must experience a qualifying event in order to become insured. How do I enroll in insurance after I waived coverage?

How do I enroll in insurance for the first time?
If you are new to NKY Health and have not been in the "state" system before, please refer to the "Quick Guide: New Employees" section of the Group Health, Dental, and Vision Insurance Benefits page of this site for details.
If you are not new to NKY Health and/or you have been in the "state" system before (either as an employee or a dependent covered on another employee's plan), then you will need to work with HR to complete the "Benefits Enrollment/Change Form" found in the "Benefits Enrollment/Change Form" section of the Group Health, Dental, and Vision Insurance Benefits page of this site.
Aside from your personal demographic information, the exact details of what goes on the form vary depending upon the details of your prior participation. Please reach out to HR to find out what information you need and for guidance through the process.

Which plan is the best?
There is no "best" plan. You should consider your own individual circumstances when selecting a plan. Compare premium costs with coverage and deductibles against your health conditions and how much you are likely to use. The plan that might be best for you may not be best for someone else who has different health conditions.

Who is eligible for coverage?
Full-time employees and employees working 1200+ hours/year are eligible for health, dental, and vision coverage the first day of the month following their full month of employment. An employee’s spouse and dependents up to age 26 are also eligible for coverage.

How do I apply for FMLA protection?
As soon as you know you will need leave, print and sign the "Notification of Company Procedures and Employee Rights and Obligations" form, have a witness (anyone over 18) sign it, and then scan all pages into a single .PDF and email the .PDF to hr@nkyhealth.org.
If your leave is related to a medical condition (yours or someone else's), then a "Certification by Health Care Provider" form must also be completed. The patient's health care provider should complete the form and give it back to you. You should make sure that 1) what they write is consistent with your understanding and 2) that the form is complete and contains sufficient detail to predict when and for how long you will need to miss work. If you are the patient, this information should be in the "Part: B Amount of Leave Needed" section on Page 1. If you are not the patient, then this information should be in "To Be Completed by the Employee Needing Leave to Care for a Family Member" section on Page 2. When you are certain the form is accurate and complete, please scan all pages into a single .PDF and email the .PDF to hr@nkyhealth.org.
If your leave is to bond with a new child, then send an email to hr@nkyhealth.org listing the dates you plan to be off and follow up by emailing a .PDF with proof of birth or adoption as soon as you have it.
If your leave is due to a military exigency, then submit a .PDF of the official orders to HR. (Refer to How to Submit Forms/Documents to HR for detailed instructions about how to submit the document.)

Why are FMLA forms necessary?
Although they can seem burdensome for both employee and employer, FMLA forms are necessary to ensure that appropriate documentation is obtained as per guidelines established by the Department of Labor (DOL.)
NKY Health has worked with legal counsel to simplify the DOL's sample forms in order to reduce the burden as much as possible.
Thus, some employees have even been able to negotiate with health care providers to reduce any fees the provider charges for completing FMLA forms.

What holidays do I get paid for?

How much coverage does NKY Health provide?
NKY Health provides $20,000 of coverage to eligible, full-time, merit system employees.
How do I get additional coverage?
There are two ways to get additional coverage:
Increase the amounts covered under the Group Life Insurance provided by NKY Health
Enroll in a voluntary insurance plan
Neither option is better than the other. You should compare plans and choose whichever is better for you.
With the Group Life option:
Premiums are deducted from your pay on the first two pay periods of each month and paid on your behalf.
Within your first 30 days of hire, coverage is guaranteed issue up to a certain amount and subject to underwriting approval after 30 days.
Coverage over the guaranteed amount is subject to underwriting approval even within your first 30 days of hire.
You can take the plan with you if you leave NKY Health, but the premiums are likely to increase.
Refer to the "Group Life Brochure with Rates for Additional Coverage" section of the Group Life Insurance, (Includes Legacy Planning and Travel Benefits) page of this site for rates and guarantee amounts.
With the Voluntary option:
You work directly with the insurance company to arrange payment of premiums.
All coverage is subject to underwriting approval.
The plan is yours no matter where you work.
Rates do not change due to end of employment.
Refer to the Life Insurance brochure in the "Colonial Life Insurance Brochures" section of the Voluntary Personal Insurance page of the HR Site for Employees for rates and a table to help you calculate the amount you need.
How do I change my beneficiary?
You can change your beneficiary any time. Most insurance carriers have their own forms and processes for doing this.
Please refer to the "Group Life Insurance, (Includes Legacy Planning and Travel Benefits)" section the Group Life Insurance, (Includes Legacy Planning and Travel Benefits) page of this site for instructions about how to change the beneficiary for a policy you have with them.

What if I don't need coverage?
This is the kind of coverage we hope you'll never use but that is nice to have in case you do. And NKY Health pays 100% of the cost for eligible full-time employees. So, you may want to seriously think about whether you really want to decline it, but if you do, just write "waive" across the forms.
NKY Health does not pay you anything or give you anything extra for declining it.

What is the basic amount of coverage?

Who is eligible for coverage?
Full-time employees and employees working 1200+ hours/year are eligible for life and accidental death and dismemberment coverage the first day of the month following their full month of employment. An employee’s spouse and dependents up to age 19 (or 25 if a full-time student) are also eligible for coverage.

Who needs to know if I'm pregnant?
Please inform your supervisor as soon as you know you are pregnant. They will email you a notice about special employment rights you have due to your pregnancy, notify Human Resources of your pregnancy, and begin making plans for your absence related to the pregnancy.
Other than informing those who have a business need-to-know, they will keep this information confidential unless you give them permission to share it.
You may choose to tell others at your own discretion.
How does FMLA leave work work with pregnancy?
Prenatal Absences
NKY Health does NOT require the use of FMLA leave for prenatal absences related to a normal and healthy pregnancy. However, complications that require longer and/or more frequent absences, are treated as any other medical condition. As such:
If you are eligible for FMLA-protection, then you will need to complete and submit the "Certification by Health Care Provider" and the "Notification of Company Procedures and Employee Rights and Obligations" forms. (Refer to the Family and Medical Leave Act (FMLA) Protection page of this site for details and forms.)
If you are not eligible for FMLA protection, then you will need to request leave under NKY Health's regular leave policies. (Refer to Section 5 and subsections of NKY Health's Personnel Policies for details about leave types.)
Postpartum Absences
Two types of postpartum absences are protected by the FMLA:
Medical leave such as:
time to recover from the birthing process
time to care for someone who is recovering from the birthing process
time to care for the newborn due to any serious medical condition
Bonding leave which is time to bond with the newborn
About the FMLA-protected medical leave and bonding leave:
All requests for medical leave must be accompanied by both the completed "Certification by Health Care Provider" and the completed and witnessed "Notification of Company Procedures and Employee Rights and Obligations" forms. (Refer to the Family and Medical Leave Act (FMLA) Protection page of this site for details and forms.)
All requests for bonding leave must be accompanied by proof of the baby's birth (either a birth certificate or note on hospital or physician letterhead stating the parent's name and the baby's name and date of birth will suffice) and a completed and witnessed "Notification of Company Procedures and Employee Rights and Obligations" form. (Refer to the Family and Medical Leave Act (FMLA) Protection page of the HR Site for Employees for details and the form.)
Bonding leave may be taken anytime up to the baby's first birthday.
Bonding leave ends when the employee returns to work.
Medical leave can be intermittent; bonding leave must be used all in one "chunk."
You can use both leaves as long as they do not exceed 12 weeks total combined in a rolling calendar year.
If you want to take all 12 weeks as soon as the baby is born, you might find less paperwork in taking bonding leave, but if you want to split the time, then you should apply for both medical leave (now) and bonding leave (later).
NKY Health also offers Parental Leave. Parental Leave pays for the first six (6) weeks of leave due to pregnancy for eligible employees. It runs concurrent with FMLA protection for those protected by the FMLA. (Refer to Section 5.5 and subsections of NKY Health's Personnel Policies.) It is automatically approved for those approved for an FMLA protected absence due to pregnancy. Those not protected by the FMLA must request Parental Leave via email.

How much time can I take off for my baby?
If you are protected by the Family and Medical Leave Act (FMLA) and you follow the procedures for approval, you may be entitled to up to 12 weeks off work for the baby. The amount of entitlement depends on how much FMLA-protected leave you have already used within a rolling 12-month period.
If you are not protected by the Family and Medical Leave Act (FMLA), you are entitled to the amount of time that is medically required to:
recover from childbirth
care for a spouse recovering from childbirth
care for the newborn due to any serious medical condition
You may also be entitled to Parental Leave.
Is pregnancy leave paid or unpaid?
Parental Leave pays for the first six (6) weeks of leave due to pregnancy for eligible employees. It runs concurrent with FMLA protection for those protected by the FMLA. (Refer to Section 5.5 and subsections of NKY Health's Personnel Policies.) It is automatically approved for those who are approved for an FMLA protected absence due to pregnancy. Those not protected by the FMLA must request Parental Leave via email.
You may also use Annual Leave (Refer to Section 5.1 and subsections of NKY Health's Personnel Policies.) and Sick Leave (Refer to Section 5.2 and subsections of NKY Health's Personnel Policies.) to receive pay for absences related to pregnancy.

Can I retire and then come back to work?
While NKY Health would love to rehire good seasoned employees, the state legislature has strict rules that Kentucky Public Pensions Authority (KPPA) must follow regarding reemployment after retirement. The rules govern reemployment in another agency participating in any Kentucky public pension plan and some pension plans for surrounding states.
Because of these rules:
NKY Health (or any other employer participating in KPPA) cannot have a pre-arranged agreement with you to come back
You have to have a break in service of at least one calendar month before you can begin working for NKY Health or another employer participating in KPPA
If you come back within 12 months of your retirement date, both you and the NKY Health must complete forms to obtain permission to be reemployed and submit them to retired.reemployed@kyret.ky.gov before you can start the new job. (KPPA suggests allowing two weeks for a response)
Failure to obtain permission in advance from KPPA can jeopardize your pension benefits
What do I need to do to retire?
Please let NKY Health know as much in advance as possible when you're thinking about retiring then submit your formal notice when you have a final and firm retirement date. (Refer to the "How do I resign/retire in good standing?" section of this page for details.)
If you are a full-time merit system employee and/or you have ever participated in any public pension program in Kentucky, please consult with the Kentucky Public Pensions Authority (KPPA) before establishing your final and firm retirement date.
A representative from KPPA will assist you with completing all the forms you need to ensure a smooth transition into retirement. They will also give you forms for NKY Health's human resources to complete.

What retirement benefits will I get?
That depends on which plan(s) you are enrolled in.
The Kentucky Public Pensions Authority (KPPA) offers a self-service tool that you can use to estimate the amount the payments you will receive upon retirement and review options for receiving them. KPPA also offers some health insurance and burial insurance benefits to eligible retirees.

What's the difference between pension and voluntary retirement?
Participation
Participation in pension benefits, administered by the Kentucky Public Pensions Authority (KPPA), is mandatory. NKY Health is required by law to withhold a certain amount from your wages.
Participation in voluntary retirement benefits, administered by the Kentucky Deferred Compensation Authority, is completely your choice. You can contribute any amount you want and stop or start contributing anytime subject to IRS guidelines.
Contribution
More than 50% is contributed to the Kentucky Public Pensions Authority (KPPA) for every $1 you are paid. This is required by law and established by order.
NKY Health does not contribute to voluntary retirement because NKY Health is contributing so much to KPPA.
Investments
The Kentucky Public Pensions Authority (KPPA) has a board of trustees who determine how to invest funds contributed.
The Kentucky Deferred Compensation Authority provides a menu of choices for investment of voluntary funds. Please contact the Kentucky Deferred Compensation Authority for investment options.
Taxable Status
Funds contributed to the Kentucky Public Pensions Authority (KPPA) are on a pre-tax basis, meaning they are not subject to income tax when they are contributed.
Funds contributed to voluntary retirement can be on a pre-tax basis, a post-tax basis, or a hybrid basis.
Funds withdrawn from Kentucky Public Pensions Authority (KPPA) are generally subject to income tax with some exclusions and exemptions.
Funds withdrawn from voluntary retirement may or may not be subject to income tax depending upon the investment vehicle(s) you chose. Please contact the Kentucky Deferred Compensation Authority for details pertaining to taxable investments.
Loans
Both plans allow loans under certain circumstances. Please contact the Kentucky Public Pensions Authority (KPPA) or the Kentucky Deferred Compensation Authority for details pertaining to a loan.
Rollovers
The Kentucky Public Pensions Authority (KPPA) does not accept rollovers from other retirement plans. (However, it may have reciprocal agreements with other public pension plans. Please check with KPPA to find out if there's an agreement with a plan that you participated in.)
The Kentucky Deferred Compensation Authority accepts rollovers from other qualified retirement plans. Please contact the Kentucky Deferred Compensation Authority for details pertaining to a rollover.
Can I get my money back if I stop participating in the plan?
Please contact the Kentucky Public Pensions Authority (KPPA) or the Kentucky Deferred Compensation Authority for details pertaining to withdrawal of funds.

Can I work remotely?
NKY Health assigns a workstation for each employee but we allow some remote work when positions can support the flexibility. Please check with your supervisor to find out if your position can support it.
Please refer Section 3.3 and subsections of NKY Health's Personnel Policies for details about working remotely including all the details about how often you can work remotely, how far away you can be, what guidelines you must follow to obtain and retain this privilege, and how to request and obtain approval.
The request begins with an email to your chain of command with a cc to hr@nkyhealth.org. Once approved via email, be sure to complete and submit a "Remote Work Agreement" form.

How do I move from part-time to full-time or vice-versa?
You can move from part-time to full-time or vice-versa by applying for a vacant position when it is posted. If you want to move to the same classification but change your work hours, you can apply when it is posted for internal transfer. Otherwise, you can apply when it is posted for application from the general public.
Most NKY Health positions are full-time so it's easier to move from part-time to full-time but NKY Health occasionally has a business need for a part-time position.

How do I resign/retire in good standing?
We'd rather you don't go. In fact, we encourage you to let your supervisor know why you're thinking of leaving and give us the opportunity to make any changes we can to keep you.
But if you must leave us, then please email written notice to your supervisor and to hr@nkyhealth.org. The document notice should include all of the following:
Your name
The last day you will work
The reason you are leaving
Your handwritten signature
If you are a merit system employee, then the notice will be a "notice of resignation" or a "notice of retirement." If you are a contract employee, then the notice will be a "notification of early termination of contract."
NKY Health prefers that you give us as much notice as practicable, but at least the amount that is required. For the amount required:

If you are a contract employee, please refer to your contract
After you email the notice, your supervisor will send you a notice of acceptance and ask you to confirm receipt. Please watch your email and confirm receipt as soon as possible.
Don't worry. NKY Health normally wants employees to work the full term after giving notice. We typically do not escort folks out early.

What should I do if I am summonsed to Jury Duty?
Please email a copy of the initial summons you receive to your supervisor and to hr@nkyhealth.org as soon as you receive it.
Each time you report please obtain a certificate of service and email it to the following:
your supervisor
When you complete your timecard, please code the time to civil service with a note that you had jury duty. Any funds you receive for reporting/serving are yours to keep.

What should I do in the event of a name change, address change, divorce, marriage, birth of baby, adoption?
Do all of the following within 30 days, sooner if possible:
Obtain substantiating documentation and be ready to email it when requested
Send an email to hr@nkyhealth.org notifying them of the change
Update your personal data in ADP
Make any changes needed to your dependents and/or beneficiaries in your benefits plans
