There are two epic enemy accessible via either faction's fleet. The Dreadful Entity codex is no longer obtainable, and the Hateful Entity has no codex, nor are they required for any achievements.

Dreadful Amulet
The Dreadful Entity does not spawn like a normal boss, and instead must be summoned. To summon the boss you must have a Dreadful Amulet in your inventory and must activate it in the pictured location. The Dreadful Amulet drops from Nightmarish Dreadtooth and may be used every 23 hours. The Dreadful Entity may only be summoned within [VETERAN] Terror from Beyond.
Dread Guard's Corrupted Mask
Crest of the Dread Masters
The Dreadful Entity has a special passive called Dread Touch which kills players immediately. The only way to protect from this passive is by wearing a Dread Guard's Corrupted mask, which drops from Dreadtooth, or Crest of the Dread Masters, which drops from the Hateful Entity. Both headpieces are modable to keep them competitive in modern updates.
