Pre-departure information

You must login to "my study abroad" (https://app.studyabroad.illinois.edu/) and complete all required forms before departure. Some forms will be completed online. Some documents can be uploaded online or submitted in person to 2002 Lincoln Hall.

The following forms in My Study Abroad are due by November 1 (spring semester abroad) or by April 1 (fall or AY abroad):

• Signed Confirmation of Participation (COP) form--Sign and either scan or take clear photo and upload to My Study Abroad OR turn in hard copy to 2002 Lincoln Hall.)
• Pre-departure meeting will be marked complete after the event by staff.
• Electronically-signed signature documents (submitted online)
• Health and safety quiz (submitted online)
• Emergency contact (submitted online)
• Name, email and photograph release (submitted online)

Forms that may be submitted after the above deadline:

• Course approval forms – Complete with your academic advisor and submit in person to 2002 Lincoln Hall.
• Travel itinerary - Complete before leaving the country and submit online.
