Progress reports are due at intervals of 3 months (due at 3 months, 6 months, 9 months and 12 months).
The date of the report is about the most recent IFSP date (not your start date).
Fill out all areas (including attendance), and refer to the status of the child at the beginning and end of each report time frame in order to describe their progress.
Write the goal # and goal verbatim from the IFSP document.
Make reference to the frequency of treatment.
Complete as close as possible to the due date and submit to the counties ASAP. The progress reports must be filled out using the fillable form in Provider Soft.
If you are working with a Spanish speaking (only) family, reports should be written in Spanish.
Progress notes can be completed at the visit with the family using the fillable form in Provider Soft. Parents are required to sign the report. Provide parents with a copy and CIS Admin will send a copy to the SC.
Progress reports should include parent/guardian involvement, and illustrations of coaching. Progress reports should always be related to the parent/guardian concerns, and do not need to be as detailed as the Outcomes sheet. Discuss daily routines and how the family has been coached. Carry over routines to other environments such as community settings (i.e., at Grandma’s, at the grocery store, daycare or during car rides etc.). “Results and Recommendations” need to vary from time period to time period. Reports should be different from one another. They should reflect progress since the evaluation (if it's a 3 month report), or since the previous progress report.
You will be notified 2 weeks ahead of the actual due date for progress reports in Provider Soft.
Corrections to session notes (if hand-written), progress reports, etc. should follow DPW standards (one line through incorrect documentation, initial and date and correct either above or below).

TIPS:
1) Only score the area relating to the goal. For example, if the goal involves language, complete receptive and expressive language sections.
2) If a child has an emerging skill, rate them on the lower range provided on that skill. If a child is doing a skill, rate them at their age or at the highest range provided.
3) Add up all areas and divide by the #of skills to determine the developmental age. Provide parents with a copy of this report.
