Sometimes it's necessary for cases to be transferred to new EI providers for various reasons. When this happens a plan will be set in place with the whole team.
In general:
A spreadsheet will be generated by CIS Admin to include child's name, location, and units. Staff may be asked to provide additional information such as schedule restrictions
CIS will contact the SCs regarding the coverage plan (sometimes this starts at the supervisor level). It is important to come to the SC's with a solution because we do not want services to lapse for the family.
The family will need to be in the know of the transfer. Some counties prefer that the SC reach out to do this.
The departing provider must provide their last date of service to Ann
The departing staff member may tell the family the last date they are able to provide services. The EI provider should direct the family to the SC or CIS if they have any other questions.
If a case has been transferred to you, you should fill out all paperwork as you would if you were opening a case. This should be completed using the fillable initial paperwork in Provider Soft. If you are a therapist, you should fill out a new Rx in Provider Soft. You likely will not need to do entry ECO scores as the case was previously opened (but, do check to make sure there are entry ratings in Pelican (feel free to ask Ann). Assuming the outcome strategies were already entered, please check to make sure they are accurate after you meet the family. If you need to make changes to the strategies, communicate your thoughts with the SC (the record may be locked for editing, too).
