Children may be discharged from services when they have reached all of their objectives or no longer exhibit a 25% delay in the developmental area, which you are treating. A child who is turning 3 years old and who will continue to qualify for services will be transitioned to the Intermediate Unit (IU) for services. A transition report can be submitted as early as 60 days before the child’s 3rd birthday to the Service Coordinator to help the IU to accurately complete their evaluation (the Transition meeting will be scheduled by the county within 60 days prior to Transition). Providers are not allowed to attend these meetings, however, the location and time of the meeting will be shared with the provider if shared with CIS. If you are discharging prior to the child’s 3rd birthday, the Service Coordinator should be notified 2 weeks in advance as they need to contact the family and offer a “transition to the community meeting” within that time frame (per State regulation).
If applicable, complete exit Ounce/ECO (if the child has been seen for 5 or more consecutive months-calculated from the IFSP date). This can be done up to 60 days before the assumed discharge date. Please fill out the fillable ECO form in Provider Soft. Also, complete the discharge/transition paperwork in Provider Soft. The last date of service listed must be before the 3rd birthday.
After paper documentation has been uploaded to your Google Drive/Provider Soft, staff must “double check” that the upload is present, and upon confirmation you may shred the original paperwork.
Discharge Chart Audit: When we had paper files, we asked staff members to audit their own charts prior to turning them in to us for storage. We are asking you to audit your electronic files in a similar manner, but it should be much easier now. This checklist can be found in the Documents Library in Provider Soft for your reference.
