It is best to coordinate with the Service Coordinator (SC) to complete the Outcome strategies after the initial session with the family.
Goals must reflect family concerns.
Please use the Google Form to enter your strategies. Your entry will be reviewed by CIS Admin. Once approved, CIS Admin will paste your strategies into Pelican.
Use parent calendars, checklists or charts to show progress. Make goals and include ROUTINES.
Use the word “coach” when talking about family training.
Objectives should last 1 year, but should be developmentally appropriate, not age appropriate. Use the HELP Guide to write the teaching strategies.
Specifically state in the “How” section, “Progress reports will be given to the parents and service coordinator every (insert appropriate # for the county) months based on parental report and observation.”
Measurements must relate back to the goal.
If a goal number changes, please make sure the new goal information matches between Pelican and ProviderSoft.
Even if the goal is staying the same, you must make a notation at the end of the outcome goal statement section with the annual date and that the goal is still appropriate. You also need to note the date of the annual and any new changes made to the strategies in the strategies section. If the strategies are to stay the same, note “still appropriate”.
The goal will be partially written by the SC, and will also include what the provider writes as the measurable part in the "How" section as the second sentence of the goal statement. Try to focus one routine. Think about where the child should be developmentally in 6 months. The goal should be written using the "7 Aspects of High Quality Outcomes and Goals". Keep open communication with the SC if the goal doesn't seem right.
