You should be notified about the dates of the annual evaluation meetings by the county (or sometimes through CIS Admin), but you are not to attend (you will not be paid). You also may not provide services, unless otherwise authorized by the Service Coordinator (SC), on the day of the evaluation. Your 9 month progress report (and other feedback supplied to the SC) will provide insight and observations to the independent county evaluation team. This form can be found in Documents in Provider Soft. Please ensure an annual is warranted as you fill out this form. Sonja and Dennine will review it once it is submitted to Administration in Provider Soft.

If services continue after the annual evaluation--you must update the outcome page to reflect new strategies and .
If a goal number changes, please make note of this date and change the goal in Provider Soft.
Even if the goal is staying the same, you must make a notation at the end of the outcome goal statement section with the annual date and that the goal is still appropriate. You also need to note the date of the annual and any new changes made to the strategies in the strategies section. If the strategies are to stay the same, note “still appropriate”.
If services continue, there will also be a new IFSP date. Subsequent 5 and 9 month progress reports will be calculated based on the new IFSP date.
If the child does not qualify for additional services at the time of the annual evaluation you will be notified by the SC to submit applicable discharge paperwork.
