Progress Monitoring: Progress reports are due at intervals of 3, 6, 9 months and should be completed in the session with the family, preferably with the Service Coordinators. Have your reports completed 2 weeks prior to the quarterly due date and share in Provider Soft (Ann Lowell will send it on to the Service Coordinator and Supervisor). All quarterlies must be signed by the parent before sent on to Ann/the county. The date sent (with SC name and county) should be noted on your communication log.
Complete your reports 2 weeks prior to the quarterly visit, ensure the parent signs it, then upload it into Provider Soft

The date of the report is about the most recent IFSP date (not your start date)
Fill out all areas (including attendance), and refer to the status of the child at the beginning and end of each report time frame in order to describe their progress
Write the goal # and goal verbatim from the IFSP document
Note where the child started at the beginning of the outcome & where the child is currently working on this outcome
Make reference to the frequency of treatment
The progress reports must be filled out using the fillable form in Provider Soft
If you are working with a Spanish speaking (only) family, reports should be written in Spanish.
You will be notified about the dates of the annual evaluation meetings, but you will NOT be paid for your time to attend
Progress reports should include parent/guardian involvement, and illustrations of coaching. Progress reports should always be related to the parent/guardian concerns, and do not need to be as detailed as the Outcomes sheet. Discuss daily routines and how the family has been coached. Carry over routines to other environments such as community settings (i.e., at Grandma’s, at the grocery store, daycare or during car rides etc.). “Results and Recommendations” need to vary from time period to time period. Reports should be different from one another. They should reflect progress since the evaluation (if it's a 6 month report), or since the previous progress report (the 10 month should be a reflection of what happened since the 6 month report).
You will be notified 2 weeks ahead of the actual due date for progress reports in Provider Soft.
Corrections to session notes (if hand-written), progress reports, etc. should follow DPW standards (one line through incorrect documentation, initial and date and correct either above or below).

TIPS:
1) Only score the area relating to the goal. For example, if the goal involves language, complete receptive and expressive language sections.
2) If a child has an emerging skill, rate them on the lower range provided on that skill. If a child is doing a skill, rate them at their age or at the highest range provided.
3) Add up all areas and divide by the #of skills to determine the developmental age. Provide parents with a copy of this report.
As a reminder, caregiver signatures are required. If you cannot obtain their signature, please document on the session note how you shared this information with the family, and check off the box in the parent signature section of the quarterly
If you attend a meeting for a progress report, a session note is needed for that session. Please note on your billing form that the session was a meeting so we know to use "teaming code" to bill
