You have been working at Home Care Agency for a few months now and the Care Manager has asked you to attend multi-disciplinary meetings for two of the individuals that you work with. The Care Manager wants to know how the different professionals work together to meet the service users’ needs and how multi-agency and multi-disciplinary teams involved.
Produce a report to explore how the Care Agency works together with other organisations using your own role from the scenario and 2 other health or social care professionals who make up part of the multi-disciplinary team involved in the care of 2 individuals you work with.

1. Re read each case study and note any details the case study may feel uncomfortable sharing.
2. P55 cut and paste diagram and explain the working practices for managing information.
3. For each detail justify how the MDT can work together to meet their needs but also limiting the need to share every detail with every member of team.
