The Incident Report (IR) form is a way for us to document important things of note that happen during shifts. As an employee of SafeRide, it is your responsibility to inform the Coordinator of what is going on during your shift. Ultimately it is better to write an IR than to not write one. More often than not, these forms are kept in file and the only way something can be fixed is if there are recorded instances of it.
If you are unsure if you should write an IR form about something, ask your Nightly Operations Leader and they can help you decide.
Here are some reasons why you might write an IR:
You are the driver/navigator during a shift and your vehicle is in an accident.
During a shift you get a passenger who is sick with alcohol poisoning and will need further medical attention.
While driving the vehicle you notice some problems that need to be addressed (flat tire, low pressure gage light, windshield wipers not working, weird noise coming from the hood, etc.).
One of your passengers is distressed or disruptive.
You are pulled over during a shift by the police or public safety.
You refuse service to a passenger for any reason.
A passenger indicates they are harmed or in danger in any way.
An employee's behavior, job performance (driving, navigating, spotting, etc.), or attitude is cause for concern
Any emergency procedure is followed
