Wellbeing solutions and gym (fitness) reimbursement are programs from our insurance provider, Anthem Blue Cross Blue Shield, to incentivize healthy life choices via rewards.
The Wellbeing Solutions rewards program provided by Empire offers digital health and wellness tools, sponsored by Orchestra. Complete activities to earn rewards towards electronic gift cards from select retailers, with a maximum of $700 available.

Preventive Care: Includes annual wellness exams, cholesterol tests, cancer screenings, mammograms, eye exams, and flu shots. Rewards range from $5 to $25 per activity.
Condition Management Programs:
ConditionCare: Work with a health coach; earn up to $225.
Future Moms: Support for moms-to-be with up to $125 in rewards.
Well-being Coaching
Weight Management: One-on-one coaching with a $80 reward.
Tobacco Cessation: One-on-one coaching with a $60 reward.
Digital & Wellness Activities: Includes logging into an Empire account, connecting devices, completing health assessments, tracking steps, etc. Rewards vary, up to $60 for some activities.
Use the Well-being Coach digital coaching app or the SydneySM Health app.
App offers 24/7 support for various health goals.
Rewards are added to accounts upon completion of activities or post-claim processing.
Redeem rewards for electronic gift cards from various retailers like Amazon, Apple, Target, etc.
Rewards must be redeemed before the end of the plan year.
If you are enrolled in BerlinRosen’s BCBS medical plan, you can be reimbursed up to $400 annually for you and each covered dependent (18+) for things like memberships at qualified gyms, health clubs, fitness centers, ClassPass, and online/app-based fitness programs by completing 35 workouts at a qualifying fitness center every six months within the benefit year.
The program is open to Empire members who are age 18 and older. This includes you and your covered dependent(s).
The most you can be reimbursed each year is $400 for yourself and $400 for your covered dependent(s).
If you become eligible or add a new dependent after the effective date of your group’s benefit plan year, you and your dependent(s) can still take part in the program. Your workout requirements and reimbursement will be based on the number of months you are eligible.
You must have Empire health coverage with BerlinRosen through the date on which you can be reimbursed.
Memberships at qualified gyms, health clubs, and fitness centers, as well as subscriptions to qualified online and app-based fitness programs are eligible.
To be reimbursed, each participant dependent needs to work out at a qualifying fitness center 35 times in each six-month period within the benefit plan year.
Gym reimbursements can only be submitted twice a year, each covering a 6-month period. Be sure to submit your claims within each period to stay eligible. For the final 6-month period of the plan year, you have an additional 90 days after the plan year ends to submit your claim.
If you're a member of our health insurance through Anthem Blue Cross Blue Shield, you're eligible for fitness reimbursement. To get started, fill out the fitness reimbursement form. Please note the following important details:
You must submit the form twice a year:
Once for classes or gym sessions from January 1 – June 30
Again for classes or gym sessions from July 1 – December 31
You need to complete at least 35 classes in each half of the year (a minimum of 70 classes for the full year within the specified timeframes).
You can receive up to $400 per year ($200 for each half), but you must submit the paperwork twice—once for each half of the year.

Fill in your full name.
Have your fitness center complete the rest of the form. If you're using a traditional gym, a representative must sign it.
Submit the completed form along with proof of payment and proof of attendance (including for virtual classes).
You'll receive an automated acknowledgment after submitting.
