The etiology of shock in pediatric patients is usually due to hemorrhage, dehydration, or toxic ingestion. Shock should be actively treated if the following signs/symptoms are present:
Decreased level of alertness
Tachycardia > 180 bpm
Capillary refill > 3 – 4 seconds
Decreased systolic & diastolic pressures
Cool, clammy skin
Normal systolic blood pressure in the pediatric patient is calculated as follows:
70 + (age in years X 2)
Follow General Pediatric Protocol.
Mandatory interventions:
a. Oxygen
b. IV or IO access
c. Cardiac monitoring
d. Rapid glucose testing
Infuse a Isotonic Solution (NS or LR) 0.9% NORMAL SALINE 20 mL/kg fluid bolus.
a. May repeat every 20 minutes as needed for adequate response as manifested by:
i. Increased level of alertness
ii. Decrease in HR
iii. Increase in SBP
