NO IEP/504:
If the Special Programs column is absent of any IEP or 504 indicators, and there is no mention of other applicable ESE documentation in the Shared Comments section of the student’s Contact Login VSA, confirm with the family that the student does not have an IEP, 504 Plan, or any other applicable ESE documentation.
Be sure to LOG THIS CONVERSATION IN VSA.
SELF-REPORTED:
If the Special Programs column includes an IEP or 504 indicators, confirm with the family that they have self-reported an IEP/504 Plan. Provide the family the ESE Team email and fax information so that they may submit necessary documentation.

IEP/504 ON FILE WITH FLVS:
If the Special Programs column includes an IEPor 504 indicator, confirm with the family that FLVS has received, verified and filed the IEP/504 Plan (this information will also be present in the Shared Comments box in the student’s Contact Log in VSA). Initiate discussion to learn about the student’s disability/impairment and their strengths, abilities and needs in each course.

Contact the grade level ESE Specialist for assistance in understanding how to appropriately implement accommodations and strategies to support the student as needed.
If the Shared Comments box in the student’s Contact Log in VSA indicates that other applicable ESE documentation has been received, confirm such with the family. Initiate discussion to learn about the student’s strengths, abilities and needs.
