On 18th November 2017, Mark Milsome was killed on a film set in Ghana whilst operating a camera for a stunt that went tragically wrong.
An Inquest held at West London Coroners Court found that Mark died an accidental death. The Senior Coroner went on to say that "Shortly before the execution of the stunt, the risk of Mr Milsome being harmed or fatally injured was not effectively recognised, assessed, communicated or managed".

In October 2021, the Mark Milsome Foundation Film and TV Online Safety Passport Course was launched with 94% of crewmembers surveyed saying they would voluntarily enrol on the course if it became recognised as an industry standard.
