REDCap – Data Management & Monitoring
Pur Umeed Zindagi (PUZ) utilizes REDCap (Research Electronic Data Capture) as its primary digital data management system to ensure accurate, timely, and secure documentation of all mental health services delivered across sites.
Daily Data Entry by MHOs
All Mental Health Officers (MHOs) are responsible for daily data entry on REDCap. Every individual interaction is documented in real time or at the end of the service day to maintain data accuracy and continuity of care.
The following information is routinely entered into REDCap:
Screening data
Initial screenings (PHQ‑2, GAD‑2)
Detailed assessments (PHQ‑9, GAD‑7, MANSA)
Enrollment details
Client registration and informed consent status
Assigned site, MHO, and care pathway
Counseling and follow‑up sessions
Session dates
Type of intervention provided
Progress notes and clinical observations
Referrals
Referral to psychologists, psychiatry, or other services (when required)
End‑of‑Treatment (EOT) data
Final assessment scores
Outcome status and case closure details
Standardized & Structured Documentation
REDCap forms are designed using standardized templates aligned with PUZ clinical protocols. This ensures consistency in data collection across all regions and sites, regardless of location or service volume.
Monitoring, Supervision & Quality Assurance
Data entered in REDCap enables:
Clinical monitoring by psychologists and regional leads
Case tracking from screening to end of treatment
Outcome measurement and service effectiveness evaluation
Program reporting using de‑identified and aggregated data
Supervisors regularly review entries to ensure data quality, clinical accuracy, and ethical compliance.
Confidentiality & Data Security
Access to REDCap is role‑based and restricted to authorized PUZ staff only. All client information is stored securely and handled in line with IHHN privacy, confidentiality, and ethical standards. Data is used solely for clinical care, supervision, reporting, and approved research purposes.
Supporting Evidence‑Based Decision Making
By maintaining a centralized and reliable data system, REDCap supports PUZ in:
Identifying trends in mental health needs
Improving service delivery and follow‑up care
Strengthening accountability and transparency
