INSTRUCTIONS ON ACTUAL COMMUNITY HEALTH MANAGEMENT

Identify one barangay near your city or make use of a community assigned to you.

Make a situational analysis.

Analyze the health problems.

Make a community diagnosis.

Prioritize the health problems.

Formulate a plan of management (or treatment).

Formulate a HFA/COMMUNITY or BARANGAY/5-10-15 Years Program. (HFA – Health for All)

Formulate a plan on how to assess the result of management (or treatment).

Formulate a plan on how to prevent the recurrence of the problem.

Feel free to consult medical textbooks to help you in accomplishing your project.

Report to the group and to the faculty your accomplishment.

Demonstrate to peers and to the faculty your accomplishment of the learning objectives.
