INSTRUCTIONS ON PERSONAL HEALTH MANAGEMENT

Analyze your own health status.

If you have a health problem,

Make a diagnosis of your health problem.

Make a plan on treatment.

Make a plan on how to assess the outcome of treatment.

Make a plan on how to prevent recurrence of your problem.

If you don't have a health problem,

Make a plan on how you will maintain your health.

Report to the group and faculty.

Demonstrate to peers and faculty your accomplishment of the learning objectives.

INSTRUCTIONS ON PERSONAL FAMILY HEALTH MANAGEMENT

Analyze the health status of your own family.

If there is a health problem,

Make a diagnosis of your family’s health problem.

Make a plan on treatment.

Make a plan on how to assess the outcome of treatment.

Make a plan on how to prevent recurrence of the problem.

If there is no health problem,

Make a plan on how you will maintain and improve the health of the members of your family. Make a health promotion program for your family, one that is feasible and acceptable to the members of your family.

Report to the group and faculty.

Demonstrate to peers and faculty your accomplishment of the learning objectives.
