This Section Should Only Be Used by PSYCHS, SLPS, OTS, PTS or CAT.
Eligibility Categories and Assessment
Assessments are preselected below based on the eligibility category.
Others may be selected if desired.
If you are not reevaluating to change eligibility, you can uncheck all the suspected disabilities. This will give you the freedom to select (or de-select) any assessments you want.
Assessments are automatically selected based on which disabilities are being considered.
If you are not re-assessing current disabilities, you can uncheck them, and then the required assessments for that disability will go away.
Create the Written Notice
The system will create the appropriate Notice depending on what you said the Purpose of the Meeting was.

Reevaluation
Once the Notice has been signed by the parent, it must be sent to the Special Education Administrative Assistant and psychologist (or SLP), or directly downtown (for an OT/PT/CAT only evaluation). The date the consent was received will be registered in IIEP, and the signed Notice will be uploaded on the Documents page.
