Exit IEPs

Scheduling the Exit IEP

You must hold an Exit IEP during the last semester or quarter of a student's secondary school experience. This way, issues such as course credits, graduation status and post high school needs can be best addressed. However, the Annual CC may also be the Exit IEP. The IEP may need to be revised if the student's graduation status changes.

A Transition Rep, such as VR or BDDS, must be invited to the Exit IEP. Parent consent must be obtained prior to the exit conference to invite these individuals, and should be noted in the Documentation of Presentation of Adult Services Materials text box on the Provisions page. It is recommended that permission be asked for at the Junior year conference.

If the student is 18 years or older, and is emancipated (or is an emancipated minor), you will need to add the student under Parent Info, and check the box for "Legal Guardian"

Purpose

  • You will select Exit IEP as the Purpose when:

      • The student is graduating with a diploma during the effective dates of the IEP.

      • The student will turn 22 during the effective dates of the IEP.

      • The student is graduating with a certificate of completion and will not, or is unsure if they will attend YAS.

      • Student is exiting from Project Search.

  • You will NOT select Exit IEP as the Purpose when:

      • The student is graduating with a certificate of completion and intends to attend YAS.

Effective Dates

  • If Exit IEP is part of an ACR, beginning date would be from end of the birthmonth to the end of the school year

  • If Exit IEP is after the student's ACR, the beginning date will be the date of the conference to the end of the school year

  • If the student's birthdate is in the summer, the beginning date will be July 1 until the end of the school year.

  • Do not check Revise IEP as a Purpose in addition to Exit IEP, or you will not be able to change the Effective Dates.

  • An IEP must be written for all students, including those who are exiting high school and/or Young Adult Services (YAS). Please include the Anticipated Services form with the IEP.

  • If the student is graduating with a diploma, please complete the Permission for Termination of Services Due to Graduation Form.

Goals

  • Write goals for the student for the remainder of their time at SBCSC. These goals should focus on the specific skills the student is working on to prepare for their future within the time left in SBCSC.

Provisions

  • Transition Service Activities

  • If the student is planning to pursue postsecondary education, a Transition Activity could be to encourage the student to contact the student services department at their chosen school.

  • If the student is planning to join the workforce following high school and/or YAS a transition activity could be for the student to contact the office of Vocational Rehabilitation.

  • If the student will need in-home or community supports, a transtion activity could be for the student and family to contact the Bureau of Developmental Disabilities.

Special Education Services

Special Education Services should include the typical student's schedule for the rest of the student's time in SBCSC. Services should be written for the current setting, with an ending date of the end of the IEP.

Summary of Performance

A Summary of Performance is always required as part of an Exit IEP.

Please note that the Indicator 14 letter must be finalized before you will be able to finalize an Exit IEP.
