Meeting Purpose


Identify the Purpose of the Case Conference Meeting:

Setting the purpose of the case conference is very important, and will affect which sections of the IEP you may be able to access.

If you check Initial or Reevaluation Review, an additional section will appear below.

If the IEP is following an evaluation, select Reevaluation Review.

If this is an Annual case conference, then the option to Revise without a meeting will not be available.

If the student is returning from ANY non-SBCSC school (including local charter schools), you must select Move-In.

If you are recommending Homebound Instruction, select Out of School Placement-60 day review.

If you are considering sending a student to APPROVE, select Interim Alternate Educational Setting.

Each student MUST have an annual IEP at least every 12 months.

Explanation of the Purposes

Initial Evaluation

This purpose will only be selected by a psychologist or SLP who is doing the initial testing.

When this is selected, a multidisciplinary team member is required in lieu of an instructional strategist. Additionally, the RTI question is prompted, procedural safeguards are available and information is required for the Written Notice Proposal (to be completed by a psychologist or SLP. This is NOT the notice of case conference).

Initial Expedited Evaluation

This option is typically selected as part of an MDC when testing is needed to determine eligibility. This would only be selected by a psychologist or SLP.

Reevaluation Review

This option will be selected when you are convening a case conference to consider the results of either a psychological, OT, PT, CAT or Speech evaluation.

When reevaluation was requested in a previous case conference, the date of this conference is used to document compliance with the evaluation time line.

Annual

Annual must be selected for a student at least once every 12 months.

  • SBCSC does an annual case review each year during the student's birth month to review progress and plan for the up-coming school year. The dates to be used are located on the Special Ed Home Page.

When a student with a recent Indiana IEP moves into our district, SBCSC will import the student's IEP from the sending district. This includes local charter schools, as well as Indiana On line Academy. Follow the instructions for enrolling the student.

For a Move-in conference:

  • You will not see "Move In" as an option until after the student's eligibility and date of last IEP have been entered. This will need to be done by a Support Team member.

  • The conference must be held within 10 days of the student's enrollment

  • Use whatever information you have.

  • If you are waiting for the IEP from the previous Indiana school to be imported into IIEP, DO NOT enter information into the database. When the IEP is received from the sending school, your information will be overwritten, and lost.

  • Finalize the IEP and forward to the Special Education Office as soon as possible.

One additional note about the Move-In purpose:

  • Selecting Move In will open up certain fields of the eligibility section that are otherwise unavailable. If you need to make changes to the Eligibility, or add assessment information in the text box:

      1. Select Move In as the purpose

      2. Click Save

      3. Go to the Eligibility Section

      4. Add the necessary testing information

      5. Click Save and Continue

      6. Go back to Purpose

      7. Select the correct purpose for the IEP

  • This process is ONLY to be used to update testing information. Eligibility may never be changed on a student's IEP without holding a case conference with a psychologist in attendance.

Select when the student is up for expulsion or has reached 10 days of suspension.

This prompts the new questions under special considerations and requires components of the behavior intervention plan if appropriate.

Revise IEP

  • Use this option any time a change is made to the current IEP

  • There is no limit to the number of revisions that can be done within the IEP dates set at the annual case conference.

  • Document discussion and reasons for changing the IEP in the Notes section. If you do not delete the notes from the prior IEP, please date and clarify your additions.

  • Follow the same procedures for finalization and distribution that you would for any other IEP.

  • The student is graduating with a diploma during the effective dates of the IEP.

  • The student will turn 22 during the effective dates of the IEP.

  • The student is graduating with a certificate of completion and is unsure if they will attend YAS.

  • The student is exiting Project Search.

Consider Placement in Alternative Program

This requires the presence of a representative.

Consider Placement at a State School

This requires the presence of a representative.

Consider Placement at a Private Facility

This requires the presence of a representative.

This requires the participation of a representative from the nonpublic school.

You will select this when:

  • Student attends a non-public school

      • SBCSC Parent rejects FAPE to place their student in a South Bend non-public school.

      • Student lives out of district but attends a South Bend non-public school.

  • Student is homeschooled

  • Student is attending ABA (student is considered homeschooled because they are not attending SBCSC for any part of the school day)

PA placement with different PA of service

Although an unusual circumstance, sometimes a Public Agency (PA) places a student in a private facility that receives educational services from a different public agency. This requires an invite and the attendance from a representative from the PA of service.

Interim Alternative Education Setting

Dashboard timers will be set.

Out-of-School Placement-60 day review

Use this option when you place a student on Homebound.

Revise IEP without a Meeting

Use when parent agrees to minor changes in the IEP, or correction of errors.

  • This option is NOT available if the IEP is an Annual Case Conference

  • You may use this option when making minor changes to the IEP

  • You must document the parent contact under the Communication Section

  • Some occasions where this is a good choice:

      • Change the setting for services (i.e. the IEP says pullout services, but the student can remain in GE and have their needs met)

      • Add/Remove accommodations

  • Document discussion and reasons for changing the IEP in the Notes section. If you do not delete the notes from the prior IEP, please date and clarify your additions.

  • There is no need to attach a sign in sheet or Medicaid form

  • Send the IEP home for parent signature

Post-Secondary Transition

Will be automatically selected in the student's 14th year, and all subsequent years.

If the student is going to be 14 at any point within the effective dates of the IEP, Post Secondary Transition will automatically be selected.

If the student will be going into 8th grade at any point or will be turning 14 within the effective dates of the IEP, then you must check this box if it has not already been checked.

Reset Message

This message appears on the Purpose page:

When you check the box off and save, at midnight the data listed below will clear.

If Annual is selected as a purpose:

  • Clear out meeting dates on Schedule CCC Meeting Info Page

  • Clear out IEP begin date and end date

  • Clear out Invitees from CCC Meeting info Page

  • Clear out all green check marks

  • Clear out all dates for goals and services

  • Add ROC that dates must be within IEP dates

  • On transition page clear out the text box labeled “Document the review / discussion that occurred to determine that the goals listed above are appropriate and measurable”

  • Clear all notes

If Revise IEP or Revision without Parent Present is selected as a purpose:

  • Clear out meeting dates on Schedule CCC Meeting Info Page

  • Clear out IEP begin date

  • Clear out all green check marks

You will NOT lose information you have entered into fields in the IEP except the dates listed above.
