Due to limited inventory, school supply requests will be by referral only from campus counselors. Email Justin Eatherly, jdeatherly@forneyisd.net the student's name and campus and the supplies will be delivered directly to your campus. Reach out to Justin with any questions or urgent needs.
The goal is to ensure that no student starts school without the basic supplies they need but also make sure counselors are aware of the situation and can mediate any issues. By streamlining requests through campus counselors, they can better assess the greatest needs and respond as quickly and efficiently as possible.
They will continue doing everything they can to support our students, and theyappreciate your help in coordinating this process on your campuses.

Counselors can go to the First Presbyterian Church (12433 FM 1641, Forney) and shop during those hours for your students.

Use this link to submit counts for weekly food bags and again when / if you need to make changes to your weekly food bag count. If you don't have any changes for the week, you do not have to resubmit the form, you will receive the same amount of food bags as the previous week. The deadline for changing your food bag count is 2pm every Tuesday. This calendar notification will appear on your calendar every Tuesday.
Students that enroll at your campus Wednesday-Friday in need of this resource will not receive a food bag until the following week. If a special situation occurs and you need it before then, call me and I will try to get you what you need.
Hygiene kits from First Baptist Church (COMPLETE THIS FORM BY THE FIRST OF EVERY MONTH)
Use this link to submit counts for students to receive a personal hygiene kit specifically for boys or girls at the beginning of each month.
First Baptist Church will deliver the hygiene kits to the student's home or campus, depending on the parent/guardian's preference. If FBC is delivering the kit to the student's home, a Parent permission form must be completed by the parent/guardian giving FISD permission to release the student's confidential information to FBC. Keep the signed release of information with your records.
Christmas Assistance / Angel Tree for Forney ISD Families
Deadline - November
Christmas assistance sign-ups will begin when we return from Fall Break, October 21 - November 15.
Angel Tree is open to children ages 0-18 who are on free or reduced lunch or have experienced a hardship in their immediate family this year. We DO NOT mass email or share this out, as families must qualify to be eligible. This is in partnership with the Lions Club Angel Tree program, so families only need to sign-up once (with either their counselor or during the Lions Club public sign-ups.) After families sign up, they will receive an email after Thanksgiving with their assigned pick-up day, time and location, probably December 17-18.

If needed here is a paper copy - Print the form to send home with the student. If this form is not returned, call home to get the information from the parent/guardian.

Alexis Alvarez will be available to assist with applications every Monday from 9:00 AM to 1:00 PM at our office, located at 800 Ed Hall, Kaufman, TX 75142.
Additionally, Alexis is available by phone or email for any questions or assistance you may need throughout the week. We encourage you to reach out to her for support or refer anyone in need of assistance.
