This page will house information on Homebound guidelines for General Education Homebound (GEH), Compensatory Education Home Instruction (CEHI), and Special Education Homebound.
Students who are chronically ill, or who have other medical conditions which result in the student being confined to their home or hospital bed for at least four consecutive or cumulative weeks during the school year may be eligible to receive instruction at home. This placement requires documentation from a physician indicating the type and severity of the condition as well as the anticipated length of confinement to the home.
Students in both special education and the general student population may be eligible for homebound services. Special education homebound instruction is provided for IDEA (Individuals with Disabilities Act) eligible students who the ARD (Admission, Review and Dismissal) committee determines should be served at home. Accommodations for students who are in general education and have qualifying medical conditions should be made under General Education Homebound (GEH) guidelines.
When a Forney ISD student needs homebound services, the At-Risk Services Specialist, Nicole Carranza, should be notified immediately by the following campus contact person:
Campus Counselor/504 Chair—General Education homebound
Campus Nurse—Compensatory Education Home Instruction
Campus Diagnostician—Special Education homebound services
Upon notification, the At-Risk Services Specialist will coordinate with the virtual campus principal if virtual courses are appropriate or assign homebound instructor/s for the required grade level and/or subjects. The campus attendance clerk, district PEIMS coordinator and campus contact will be notified of the homebound schedule once it is set.
1. Any student who is served through the General Education Homebound (GEH) program must meet the following three criteria:
Is expected to be confined for a period of time totaling at least four weeks during the school year. This time need not be chronological. Special situations that arise due to chronic, long-term needs and will be handled on a case by case basis;
The student is confined at home or hospital bedside for medical reasons only;
The student’s medical condition is documented by a physician licensed to practice in the United States. (*If you are unsure if the signature on the verification form is that of a licensed physician, please call At-Risk Services, 469.762.4100)
2. A campus 504 committee must convene a meeting to consider homebound services and Section 504 eligibility. The committee should include, but is not limited to, a campus administrator, a teacher of the student, nurse, and a parent/guardian of the student.
3. The role of the committee is to review and consider the necessity of providing instruction to a general education student at home/hospital bedside. If instruction is provided at home/hospital bedside, the committee will determine the type(s) and amount of instruction to be provided.
4. In making these decisions, the committee must consider the physician’s information. However the physician’s note/information is not the sole determining factor in the committee’s decision making process.
5. A student served through GEH at home must be served by a certified general education teacher, and must be provided instruction in all courses, including elective courses, in which that student is enrolled.
6. The committee must meet again to consider the student’s return to school or the need for transition services.
7. Documentation must be completed indicating the day(s) General Homebound Instruction started and stopped.
In qualifying a student for and serving a student through General Education Homebound, the following documentation requirements must be met by a committee of knowledgeable persons:

1. The following forms should be sent home notifying the parent of the meeting:

Section 504 Homebound Needs Statement*
2. The Section 504 Homebound Needs Statement* must be completed by a licensed physician stating the student has a medical condition which requires the student to be confined at home/hospital for a minimum of four weeks.
3. The committee must meet and complete the Section 504 General Education Homebound (GEH)Supplement Form* regarding decisions as to whether or not a student is to be served through General Education Homebound.
4. The committee must also consider Section 504 eligibility and complete any necessary Section 504 paperwork*, if the student meets the eligibility requirements.
5. A GEH committee meeting must be held to consider a return to campus on a full-time or transition basis. A physician’s release must be obtained for the student to return to school, including times to take state assessments. The General Education Homebound (GEH) Committee Transition Services and/or General Education Homebound (GEH) Exit Documentation forms should be used.
6. The number of days homebound instruction is to start and stop must be documented.
7. Copies of all completed documents must be provided to the parent(s).
A student receiving GEH services who returns to his or her campus to take required state assessments must have a medical release from a licensed physician to do so. The time spent on campus taking the required state assessments cannot count as any part of the number of hours of GEH services for eligible days present.
