This form must be completed on every new student:
New Student PEIMS Coding Form (save as a bookmark)
Complete the google form when you receive records on all new students that enroll at your campus. .
When / if you receive more records, complete the form again with the updated information.
The appropriate contact will be notified by email of a student that requires services, ex. SpEd, Section 504, GT, etc.
A confirmation email will be sent to you with the information that was submitted.
The information you submit will be saved as a Laserfiche report.
There will NOT be a Google Spreadsheet with responses.
As a reminder, this is the counselor's responsibility and one of our non-negotiables.
