①窓口で受け取る場合 / If you pick it up at the counter

受け取り場所：所属学院の教務担当窓口

Pick up location: Academic (Student) Affairs Office of your graduate school

月曜日17時迄の申請分：水曜日に各部局へ発送（金曜日以降、受け取り可能）

水曜日17時迄の申請分：金曜日に各部局へ発送（翌週火曜日以降、受け取り可能）

※各部局への発送日が祝日の場合は、次の平日に発送となり、受け取り可能日についてもその日数分遅れます。

※時間に余裕を持って申請してください。

※受け取り可能になった旨の連絡はしませんので、上記スケジュールを確認の上、直接各部局の教務担当窓口でお受け取りください。

※配送状況により、到着が遅れる可能性があります。確実に受け取るためには事前に各部局の教務担当に確認することをおすすめします。

学生証を提示したうえでお受け取りください。

*If it is a public holiday, it will be processed on the next weekday and the day when it will be available for pick up will be delayed.

*Please apply well in advance.

*We will not notify you when your certificate is ready, so please check the schedule above and directly visit the academic affairs office of your graduate school.

*Arrival may be delayed due to delivery conditions. To ensure receipt, we recommend that you check with the academic (student) affairs office in advance.

Please bring your student ID card when receiving.

②郵送で受け取る場合 / If you would like to receive it by postal mail.

フォームでの申請と合わせて、下記2点を送付してください。

In addition to the submission of the Google Form above, please send the following two items.

・学生証コピー / Copy of your student ID card

・返信用封筒(宛先記入、切手貼付。証明書１通の重さは5g程度ですので、封筒の重さと合わせて必要な郵便料金を計算してください。) /Return envelope (addressed and  stamped, one certificate weighs about 5 grams, so please calculate the postage required along with the weight of the envelope.)
