Permission Slips

Hi all

Apologies in advance for the bulk email but I was wondering if anyone has a pro forma template that they send out to parents regarding watching horror films in class? With many horror films falling in to the MA+ classification, I would need to ask for permission from the parents of Year 10s in order to show snippets from films.
