The record of observations shall include for each object:
Specifics of the observer's site.
Date and time of the observation.
Conditions including seeing, transparency, and the darkness of the site, to include the degree to which the moon interferes with the observation.
Telescope used including aperture and focal ratio.
Eyepiece and magnification information.
Filters used.
A detailed description of the object that includes at a minimum:
Is the central star visible?
Is a filter required to observe the PN?
How does the PN respond to different magnifications?
Is the object visible by direct vision, or does it require averted vision?
A detailed description of the object's appearance in the observer's own words, OR a detailed sketch of the object.
Optics:
Celestron 14 is 14" f/11 SCT with a 3900 mm focal length (ASEM scope at Broemmelsiek Park)
SkyWatcher 12" is a 12" f/5 Newtonian with a 1500mm focal length

Seeing Scale:
5. Perfect seeing, without a quiver.
4. Slight quivering of the image with moments of calm lasting several seconds.
3. Moderate seeing with larger air tremors that blur the image.
2. Poor seeing, constant troublesome undulations of the image.
1. Very bad seeing, hardly stable enough to allow a rough sketch to be made.
Transparency Scale:
1 :: Mostly Cloudy
2-3 :: Hazy; 1 or 2 Little Dipper stars visible
4 :: 3-4 Little Dipper stars; Milky Way not visible
5 :: 4 Little Dipper stars; Bright parts of Milky Way visible (Scutum star cloud)
6 :: 5 Little Dipper stars; Milky Way visible with averted vision
7 :: 6 Little Dipper stars; Milky Way visible
8-9 :: Excellent: 7 Little Dipper stars; M-31 visible
10 :: Superb: M-33 and/or M-81 visible
