It is a universe where it is mostly centered around leeches. It is unknown what is is mostly here, but it is described as a dark gloomy place with most of the land being water, implying it is an aquatic world. Even so, most of the actual land above the ocean still has leeches all over it. Some have grown enough to become as sentient as humans, like Hiru and Ruriko.
