APPLY FOR MATERNITY LEAVE?
Getting ready for a new member of the family is an exciting time. To help you navigate through the decisions you will need to make related to your job, we have prepared this reference:
FMLA Leave
FMLA Leave is available to employees who have been employed with JUUT for 12 months and worked at least 1250 in the previous 12 months.
· The benefit is unpaid. The employer must return you to your job or similar job at the end of the leave.
· The employer continues to pay its portion of pre-tax benefits. Employees will be given the option to repay their portion of benefits deductions.
· To apply for leave, complete the FMLA information form.
o Human Resources enters the leave in ADP
o ADP will request additional information such as a doctor’s certification. Respond to all ADP questions timely as this could impact your eligibility for FMLA leave.
o Be sure to contact ADP or Human resources if your dates of leave change and when you have returned.
Short Term Disability
Contact your Short-Term Disability carrier. If you signed up with Colonial Life – contact them at 800-325-4368 or online at www.coloniallife.com.
Qualifying Event
Birth of a child is a qualifying event and any changes to benefits and beneficiaries should be considered at this time. Contact ADP to initiate a Qualifying Event such as adding the child to JUUT’S health insurance. This must be done within 60 days of birth.
Other Options
Employees who don’t qualify for FMLA may be eligible for a General Leave of Absence with their manager’s approval. A general leave does not require JUUT to maintain your job status, but JUUT will work to find the best solution that works with your needs.
