The termination form is to be completed by managers upon termination of employee. Once form is filled out and submitted it will automatically be sent to all departments that need to take action. The employee will be removed from all systems and email groups. They will be sent a termination letter outlining the details of their departure and what they can expect including final pay date with PTO payout amounts.
ADP will also send them communication on how to access their account and W2 information after termination. If the employee carried health benefits while employed with JUUT their benefits will come to end the last day of the month they are terminated. They will be sent COBRA information from ADP via mail to address on file in ADP.
Please ensure the employee has updated their personal information in ADP, including address, email address, and phone number in the case they need to be contacted after termination.
