Required Forms

Current Required Forms Status - As of Date @ Time

Required forms: There are four required forms (and one optional) outlined below that must be completed in order for you to participate in BCO and in any NJROTC Team/Field Trip or activity to include PT which is part of your grade. You must complete and turn these in to get the full benefits and experience of being an NJROTC cadet. It is a good idea to scan and save copies of your completed forms.

Form 1 - Health Risk Screening Questionnaire

Form 2 - Pre-Participation Physical Form

Form 3 - Standard Release Form

Form 4 - Personal Data Form

Follow the instructions in the boxes below for these forms

Once these are filled out/completed, bring in Forms 1, 2, and 3 to the Hurricane NJROTC Admin Department as directed below (Form 4 is submitted online):

NS1's/Freshmen - Bring these forms during your Uniform Fitting or Day 1 of BCO
NS2's/3's/4's - Bring in these forms Day 1 of BCO if you are Cadre, everyone else bring in Day 1 of school

Health Risk Screening Questionaire

Step 1: Download the Health Risk Screening Questionaire

Step 2: Fill it out (only after you have downloaded the document and opened it from your own computer)

Step 3: Print it - double sided please

Step 4: If you answered "No" to everything, just turn the form in, if you answered "Yes" to any questions, take the form to your doctor (do this when you do your physical and get both forms done at once) and have your doctor fill in the bottom section. Then turn the form in to HHS NJROTC Admin

Pre-participation Physical Evaluation

Download the form found here, or at this link (Physical Form)
Fill it out (only fill it out after you have downloaded the form and opened it from your computer or print it first and then fill it out)
Print it
Take the filled out form to your family doctor to complete.

Note 1 - If you answer "Yes" to any questions on your Health Risk Screening Questionnaire (form 1 above) take both of these forms to your doctor at the same time to save a trip.
Note 2 - If you are on a JV or Varsity team this is the same physical form you turn in to the Athletics Director (Mr. Gross) just make a copy of it and bring NJROTC a copy too.
