The application for make-up examination only applies to CURRENTLY ENROLLED STUDENTS who have failed to take the exam/s on the scheduled date for a valid reason (refer to the academic calendar for deadlines).
Application Requirement:
Accomplished Application Form for Make-Up Examination.
How to apply for Make-Up Examination:
Step 1: Submit/Email the filled out application form for make-up examination to the respective Faculty of Study (FEd, FICS, or FMDS); copy furnish the Records Section at records@upou.edu.ph.
Step 2: Wait for an email from the Faculty of Study for further instructions.
