Registration Forms 2023

Our registration process is now online with the exception of payment (unless you chose to use Chase QuickPay or Zelle).  If not using Chase QuickPay or Zelle, once you have completed the forms below, please send your payment to Kyle Perry at 505 Washington Blvd. Oak Park, IL 60302 or bring your payment (cash or a check made out to Windy City Sports) with you to the first day of practice or your first game.

The Medical Treatment & Emergency Contact form (first file below) must be completed before each season online by the first day of practice or sooner for ALL members of Windy City Sports' Water Polo Teams.

All members of Windy City Water Polo must also be members of either American Water Polo or USA Water Polo in order to practice and compete in competitions.  American Water Polo helps organize all of our local leagues and hosts some national events.  USA Water Polo organizes the Olympic Development Team and also hosts some national events.

WINDY CITY SPORTS - WATER POLO MEDICAL TREATMENT FORM — EACH SEASON, ALL ATHLETES MUST HAVE THIS ELECTRONIC FORM COMPLETED BEFORE PARTICIPATING IN ANY WINDY CITY SPORTS PRACTICES OR GAMES.  

Age Group SUMMER 2023 Water Polo League Games Registration - This form confirms your registration for the American Water Polo age group water polo summer league.  Cost is $150.  The schedule will be available shortly.  Games will take place on Saturdays and Sundays at Fenwick and in and around the Chicagoland area.  Once the schedule is finalized, you'll be able to see everything on our WCWP Calendar page.

High School Boys SUMMER 2023 Water Polo League Games Registration - THIS TEAM IS FULL - Reach out to Coach Kyle Perry if you are still interested - This form confirms your registration for the American Water Polo High School Boys Summer league.  Cost is $150.  The schedule will be available shortly.  Games will take place on Thursday nights in and around the Chicagoland area.  Once the schedule is finalized, you'll be able to see everything on our WCWP Calendar page.

High School Girls SUMMER 2023 Water Polo League Games Registration - This form confirms your registration for the American Water Polo High School Girls Summer league.  Cost is $150.  The schedule will be available shortly.  Games will take place on Wednesday nights in and around the Chicagoland area.  Once the schedule is finalized, you'll be able to see everything on our WCWP Calendar page.

High School Boys' FALL 2023 Water Polo League Games Registration - The form confirms your registration for the American Water Polo high school boys fall league.  Cost is $200.  Games will be held on Saturday and Sundays September through November.  Everything will be completed by the start of the boys' IHSA swim season.
